Documents required for moving into a facility
Whether Haus Elisabeth, Wilhelmine-Lübke-Haus or St. Albertus-Magnus-Haus: To prepare for your move into one of the senior center facilities, you can find out here in advance which documents we need from you.
To the registration:
- the application for collection
- the medical questionnaire
- if applicable, the care certificate / power of attorney / living will
- if applicable, the care classification (classification notice from the care insurance fund)
Before moving in:
- current proof of income or pension statements
- Your bank details and , if applicable, a direct debit authorization
- the self-payment declaration or confirmation from the responsible social welfare office that a social welfare application has been submitted
- the signed care home contract
When moving in:
- a copy of your identity card
- the health insurance card
- Names and addresses of the doctors treating you
- if applicable, the health insurance company's notification of exemption from co-payment for medication
- doctor's prescription for medication
Please also remember:
- Applying for full inpatient care benefits from the relevant long-term care insurance fund
- applying for short-term care benefits from the relevant long-term care insurance fund
- the necessary re-registration of your place of residence with the relevant citizens' office
- notifying the relevant health insurance company of a change of address
